What is Lung Cancer?

Lung cancer begins when abnormal cells grow uncontrollably in the lungs. These cells often start in the bronchial tubes, which carry air in and out of the lungs. Over time, the cells can form tumors and may spread to nearby tissues or distant organs.

There are two primary types of lung cancer:

  • Non-Small Cell Lung Cancer (NSCLC): Makes up about 80–85% of cases. Subtypes include adenocarcinoma, squamous cell carcinoma, and large cell carcinoma
  • Small Cell Lung Cancer (SCLC): Accounts for 10–15% of cases. This type tends to grow and spread faster than NSCLC

Diagnostic Testing and Staging

An accurate diagnosis is the foundation of effective lung cancer treatment. After identifying the type of cancer, our physicians use a series of tests to determine how far it has spread (the "stage"). Staging helps guide treatment options and assess prognosis.

Common Diagnostic Procedures

Biopsy Techniques

A tissue sample is collected for microscopic evaluation. This may involve:

  • Bronchoscopy
  • Fine-needle aspiration
  • Thoracentesis
  • Sputum cytology
  • Thoracotomy

Imaging and Staging Tests

  • CT Scan (Computed Tomography): Creates detailed cross-sectional images of the lungs and chest
  • PET Scan (Positron Emission Tomography): Detects cancer activity by measuring cellular sugar metabolism
  • MRI (Magnetic Resonance Imaging): Useful for identifying cancer spread to the brain or spine
    Bone Scan: Checks for metastasis to the bones
  • Mediastinoscopy: A surgical procedure to examine and biopsy lymph nodes in the chest

Each test plays a role in determining the cancer stage, from localized tumors to distant metastases in the lymph nodes, bones, or brain.

Personalized Cancer Treatment Based on Genetic Markers

Thanks to advances in cancer genetics, many lung cancer treatments today are tailored to the tumor’s specific molecular profile. Our team tests tumors for key genetic changes (mutations or fusions) to guide the use of targeted therapies. Targeted therapy options include:

EGFR Mutations (Exon 19 deletion, L858R, Exon 20 insertions)

First-line therapy:

  • Osimertinib (Tagrisso®) is the standard for patients with common EGFR mutations. It is also effective in treating brain metastases

For rare EGFR mutations (Exon 20 insertions):

  • Amivantamab (Rybrevant®) and Mobocertinib (Exkivity®) are FDA-approved options
  • Sunvozertinib (Zegfrovy™): Recently approved for patients who progressed after platinum chemotherapy

ALK Rearrangements

Lorbrena® (lorlatinib): Now preferred for patients with ALK-positive NSCLC. Clinical studies show it reduces progression and prevents brain metastases more effectively than earlier therapies.

Immunotherapy and Chemo-Immunotherapy Combinations

Immunotherapy plays a growing role in the treatment of lung cancer by helping the immune system recognize and attack cancer cells.

Durvalumab (Imfinzi®):

  • Approved as a maintenance therapy in stage III NSCLC following chemoradiation
  • Recently approved as both neoadjuvant and adjuvant therapy in early-stage NSCLC when combined with chemotherapy
  • Also approved for limited-stage SCLC in patients who respond to chemoradiation

These therapies may help reduce recurrence and extend survival in eligible patients.

Lung Cancer in Non-Smokers

While smoking remains the leading cause of lung cancer, up to 20% of cases occur in people who have never smoked.

These cases are often linked to factors such as:

  • Radon exposure
  • Air pollution
  • Asbestos or chemical exposure
  • Family history of lung cancer
  • Prior chest radiation
